N4’s Community Care program supports people 18 years of age and older who have intellectual and developmental disabilities, dementia and related conditions, as well as their family care partners. We do this by getting to know them and how they like to spend their time and then pairing them with someone who can do these things with them, both in-home and in the community. Families are involved in decisions and care planning.

We offer the following services:

  • Personal care
  • Social, leisure & recreational activities
  • Opportunities for civic engagement & self-advocacy
  • Transportation
  • Support accessing healthcare services, including routine wellness checks and vaccines
  • Educational opportunities for people and their family members
  • Respite for family care partners
  • Home management
  • Accessing community resources
  • Care consultation/options counseling

COVID-19 Precautions

In response to COVID-19, new precautions and services have been put in place to serve N4 Members and Community Care Partners. These include: COVID-19 health and safety screenings; home evaluations and adaptive equipment; virtual and telephone care consultation and support with accessing community resources; essential supplies, emergency kits, and education for emergency situations; food and medication delivery; and touch-free socialization, recreation and leisure.

While we continue to provide in-home support for people who need it to ensure health, safety and well-being, if a primary care partner is not comfortable having staff come into their home, we can still provide respite care. This could include taking someone for a walk, virtual socialization, getting take-out food, or community excursions that follow physical distancing guidelines.
